Community Asthma Educator – Two part time positions

This HARP-CDM funded program is an innovative community based program that aims to improve health outcomes for young people with asthma who present to the Royal Children’s Hospital and live in the northern, western and inner eastern regions of metropolitan Melbourne.

You will be part of an expanding dynamic and committed team, which provides asthma education and support to clients, families and the broader community. The successful applicant will:

Have a tertiary qualification in a health-related field
Undertake client and family assessment/reviews
Liaise and implement best practice asthma education and self-management approaches in conjunction with the client’s GP, RCH, Community Health and other Health professionals
Coordinate and propose referrals to a broad range of community, health and support services
Provide community education and support to schools, childcare, GPs and other health professionals.

These positions will be based at Doutta Galla CH (0.9 EFT) or Dianella CH (0.8 EFT) but will work with various organisations across the acute and community setting.
